1. For 2023 the Jay’s Care Foundation will be paying for your Athlete’s custom Zoom Fitting and our Winter KoolKoat and Sensory Front and Back Blanket system. (All applicable taxes, shipping using Fed. Ex Ground included, G-J Tube and Shoulder Openings for those who wear the butterfly harness for transportation.)
2. For those picking #1. all our accessories will be discounted 10% and we will do a direct invoice to the family for payment.
3. If you wish not to receive our Winter KoolKoat and Blankets Package, we will bank the dollar amount for you to go shopping for any of our other Kool Products.
4. For those picking #3 all our accessories will be discounted 10% and we will subtract the Jay’s Care Gift amount and then issue a direct invoice to the family for payment of the balance.
